Licensed Practical Nurse (LPN) – Home Visits

This is your chance to provide high-quality, personalized care—right where patients feel most comfortable: their own homes.
What You’ll Do:
Visit patients in their homes to assess health needs, monitor progress, and provide skilled nursing care.
Administer medications and treatments with precision and compassion.
Support patients with daily living activities to help them stay independent.
Partner with RNs and other healthcare professionals to create and carry out individualized care plans.
Educate patients and families on health management, medications, and wellness.
Maintain accurate and timely documentation in electronic health records.
Offer emotional support and encouragement every step of the way.
What You Bring:
Active LPN license in the state of Arizona.
At least 1 year of nursing experience (home health experience a plus).
Strong assessment, critical thinking, and communication skills.
Self-motivated, organized, and compassionate.
Current CPR certification.
Reliable transportation and willingness to travel across Tucson.
