The Disabilities Services program provides comprehensive support for neurodivergent individuals and their families. We offer information, advocacy, care management, counseling, a vibrant social club, and residential support through the Gary Shupin Independent Living Community.
We are seeking a compassionate and empathetic Care Manager to join our Disability Services team. Under the supervision of the Director of Disability Services, the Care Manager is responsible for coordinating individualized care and community support services that promote clients’ independence and well-being. This position assesses client needs, develops and implements person-centered service plans, and monitors progress toward established goals. The Care Manager also serves as a liaison among clients, families, staff, and community partners to ensure high-quality, coordinated, and goal-driven services.
Pay Range: $35 – $40 per hour (depending on experience)

Work collaboratively with the Director of Disability Services and program participants to develop and implement Individual Program Plan (IPP) that promote each client’s highest level of independence and quality of life. Support clients in setting and achieving meaningful personal and professional goals, which may include education, employment, social engagement and skill-building in areas such as personal care, budgeting, cooking, shopping, laundry, and daily living activities
Provide individualized care management and advocacy based on each client’s assessed needs, ensuring person-centered support and integration within the community
Deliver care management services to residents of the Shupin Community through regular client meetings, documentation, coordination of care with families and community partners, and linkage to external supports and resources as appropriate
Bachelor’s degree in human services, Social Work, or a related field required (Master’s preferred)
Minimum of one year of experience working directly with adults with intellectual and developmental disabilities in a residential, community-based, or social services setting
At least two years of case management or care coordination experience
CPR/First Aid certification (or willingness to obtain upon hire)
Demonstrated empathy, patience, and commitment to supporting client autonomy
Strong organizational and time management skills with attention to detail
Excellent written and verbal communication skills, adaptable to diverse audiences
Ability to maintain confidentiality, exercise sound judgment, and model professionalism
Proficiency in Microsoft Office and general computer literacy
Flexibility to work evenings, weekends, and some holidays when needed to support staffing
Must possess a valid California Driver’s License with excellent driving record. If required to drive a personal vehicle, must carry a valid automobile liability insurance meeting agency standard
